Superbills: Upon request, we can provide a superbill for you to submit to your insurance provider for possible reimbursement.
In Florida, physical therapists can treat patients without a physician referral for up to 30 days.
If further treatment is needed beyond 30 days, a physician referral will be required by law
All clients must complete an intake form and sign relevant consent forms before beginning treatment.
If you receive dry needling, a separate informed consent form is required.

We do not share your information with any third parties without your written consent
Please wear comfortable clothing that allows access to the area being treated.
If you are experiencing illness, fever, or contagious symptoms, please reschedule your appointment.
If you are pregnant, undergoing surgery, or have a new diagnosis, inform your therapist prior to treatment.
If you have animals in the home, please arrange to have them away from the treatment area during the session.
